I'm going to jump in.
The realtek is the audio chip on your laptop's motherboard. It is the audio hardware.
You are almost undoubtedly hearing the softsynth built into windows being played through it, not the Realtek soft synth.
External sound cards may improve the sound by having improved digital to analog converters compared to what is found on a chip on the motherboard and by isolating the audio circuit from other 'cross talk' within the laptop. But they also add other features including software mixers, input/ output compatible with recording devices, instrument inputs like guitar cords and midi ... to name the most important.
